- The original and photocopy of your child’s Hong Kong Birth Certificate
- Original and copy of the proof of identification documents:
- Born in China: One-way permit, Birth Certificate or other proof documents
- Born overseas: Birth Certificate, Passport or Proof of Residence
- Both Parent’s / Guardian’s photocopy of I.D. card
- Proof of Residence (a water or electric bill)
- 1 passport-size recent photographs of the child
