Please bring along

1. The Registration Form
2. The original and photocopy of your child’s Hong Kong Birth Certificate
3. Original and copy of the proof of identification documents:
   1. Born in China: One-way permit, Birth Certificate or other proof documents
   2. Born overseas: Birth Certificate, Passport or Proof of Residence
4. Both Parent’s / Guardian’s photocopy of I.D. card
5. Proof of Residence (a water or electric bill)
6. 1 passport-size recent photographs of the child
